Shipping is not showing on the 'Shipping' page of the checkout process.

I have set the shipping service up exactly as Randy Fay instructed in his video, very carefully checked it all, but I cannot get shipping options to show on the 'Shipping' page of the checkout process. Am I doing something or not doing something that prevents it showing up?

You can check it out at EveryLabel.com.au. I do need to get this sorted ... my customers are getting free shipping at present!

Any ideas on how to make this work will be well appreciated.

Does order entity contain any information about flat rate service? If not, can anybody explain relation between Order and Flat rate data?

I need it for adding information about flat rate service into notification email message.
